We made a mistake once and missed out a protection signal. It resulted in some extremely expensive crystal being damaged (melted). We should have set the system up correctly to protect this from happening but someone made a simple mistake when they got distracted. We were lucky because only equipment gets damaged when we make a mistake. If it were people instead of equipment that got hurt like the accident that happened at Alton Towers then the outcome would be much worse than a bit of a telling off.
The main thing for us is to learn from our mistakes and not do it again!

      Hi!

I worked in the lab a few times, but I did not do anything wrong. However, I was presenting to a customer once and I set-up the product wrong so that it resulted in an internal damage. It still worked – we got away with it, but it had to be fixed :P.
